Heal Strong Peptide Education

This Agreement is entered into on [Date] by and between Jared Tavasolian / Heal Strong ("Educator") and [Client Name] ("Client").

1. The Program

The Client is engaging the Educator for the following educational program (the "Program"):

Tier 1: Peptide Education Consultation
Tier 2: Comprehensive Wellness Education Program
Tier 3: Premium Educational Support Package

A detailed description of the services included in this program is outlined in "Appendix A" of this agreement.

2. Program Term

The Program shall commence on [Start Date] and will continue for the duration specified by the selected tier:

Tier 1: 4 weeks
Tier 2: 12 weeks
Tier 3: 12 weeks (initial term)

3. Program Fees & Payment

Total Fee: The total fee for the Program is $[Program Fee].
Payment Schedule: The full fee is due upfront before the commencement of any services unless a separate written payment plan is agreed upon.
Refund Policy: Due to the nature of the coaching and the immediate provision of proprietary information, all fees are non-refundable once the Program has commenced.

4. Educator's Responsibilities

The Educator agrees to provide educational services as described in Appendix A in a professional and timely manner.
The Educator will maintain the confidentiality of all personal information shared by the Client, except as required by law.
The Educator will provide educational resources, information, and wellness guidance to the best of their ability.

5. Client's Responsibilities

The Client agrees to be open, honest, and committed to the process.
The Client is responsible for implementing the guidance and strategies discussed. Results are dependent on the Client's effort and commitment.
The Client agrees to provide all necessary health information, including blood work, as requested, and to communicate any changes in their health status promptly.
The Client understands that they are solely responsible for their well-being and decisions.

6. Important Disclaimer: Not Medical Advice

The Client acknowledges that the Educator (Jared) is not a licensed medical doctor, registered dietitian, or licensed healthcare provider. The services provided are for educational and informational purposes only. The Educator does not diagnose, treat, prevent, or cure any disease or condition. The Program is not a substitute for professional medical advice, diagnosis, or treatment from a qualified physician. The Client is strongly advised to consult with their physician before implementing any health, diet, or exercise recommendations discussed in the educational program.

7. Confidentiality

All information exchanged between the Educator and Client during the Program will be held in strict confidence. The Educator will not disclose any of the Client's personal information without the Client's prior written consent.

8. Limitation of Liability

The Client agrees that the Educator shall not be liable for any damages, direct or indirect, resulting from the educational Program or any information provided. The Client's participation and any actions taken based on educational information are at their own risk.

9. Entire Agreement

This document reflects the entire agreement between the Educator and the Client and supersedes any prior written or verbal agreements.
